2010-11 Grant Continuation for KYAE Providers

To renew contracts for the 2010-11 fiscal year, providers must complete the following forms. Before completing the grant continuation forms, please read all "Important Links to Review".

Please note: The enrollment goals for 2010-11 have not yet been determined. Providers should use the enrollment goals for the 2009-10 fiscal year as a basis for their application with the understanding that these goals may be different for the 2009-10 fiscal year.

Deadline for Submission

The submitted application will consist of a grant signature page and forms.  Use the checklist to ensure you have completed all required sections.

All application materials shall be submitted electronically as described above no later than 4:30pm on May 14, 2010.

All applications must be sent by e-mail to Leah Atha.  The subject of the message should be "KYAE Grant Renewal Submission."

All required sections must be submitted together in the same e-mail message in their original file format (Word or Excel).   DO NOT SEND FORMS IN PDF FORMAT.

The original grant signature page and campaign finance law compliance form, both signed by the authorizing agent (agency head), must be mailed hard copy (in addition to submitting them electronically) to:
