Request for county program information

KYAE needs county program information for use by KYAE staff and the KYVAE/KYVU Call Center when referring adult education inquiries. 

All local adult education programs should complete the contact form (78K WORD) and e-mail it to Tammy M. Powers at tammy.powers@ky.gov by August 4, 2006.  If you have questions about the form or need help completing and sending it, contact Tammy by e-mail or phone at (502) 573-5114, ext 114.  Please remember this contact information should be updated as necessary throughout the year.

Directions for completing the form

“Contact Us” — List a staff person who can answer phone and e-mail inquiries or direct them to appropriate staff.  Inquiries include questions about GED, KYVAE, adult education, PLATO, WIN, LiteracyLink, AERIN, data entry, hours of operation, recruitment, etc.  Please arrange for a back-up person to take those calls and check e-mail when your “Contact Us” person is unavailable.  The “Contact Us” staff person and street address will be posted on the KYAE Web site and the KYVAE Web site as the main contact for your county.

AERIN/Data Contact — This staff person will be included on AERIN’s Contact Us page.  This information is used by local programs as a means of contacting other programs concerning data issues, such as separating students.  This staff person will also receive KYAE e-mail regarding AERIN.

GED Contact — List a staff person who can answer GED phone and e-mail inquiries or direct them to appropriate staff.

PLATO, WIN and LiteracyLink — List a trained instructor for each online product.  Trained instructors are those who have attended a face-to-face PLATO, WIN or LiteracyLink session. These names will be listed in KYAE’s eRMA database as the program’s main resource teacher.

NOTE: The names of all instructors trained in online products in your county appear in a drop-down list for students to select during the registration process.  The main resource instructor is always the first name listed and will receive all e-mail notifications of new registrations if no other name is chosen.  If there are no trained PLATO, WIN or LiteracyLink instructors in your county, then student registrations are linked to a default county.

Technical Contact — List the fiscal agent’s staff person who provides technical support in your county.

If none of the above sections apply to your program, fill in the name with NA.
