KYAE Online Finance Module PLEASE NOTE: DO NOT SUBMIT BUDGET AND PERSONNEL INFORMATION ON SPREADSHEETS. Information must be entered into the KYAE Online Finance Module, which will be available for entry of information after April 18, 2011.

Requirements and Deadline for Submission |

· The original Grant Signature Page and Campaign Finance Law Compliance form, both signed by the authorizing agent (agency head), must be mailed hard copy to:

· The signed, hard-copy forms must be postmarked on or before May 9, 2011.
· Budget and personnel information must be entered in the KYAE Online Finance Module on or before May 9, 2011.
· If your program design will change in 2011-12, complete the Revised Scope of Work form and e-mail the form to marcy.parker@ky.gov on or before May 9, 2011.
