2006-07 Grant Renewal Submission and Deadline

The submitted application will consist of a grant signature page and forms.  Use the checklist (28K DOC) to ensure you have completed all required sections.

Submission

All applications must be sent by e-mail to Leah Disponett.  The subject of the message should be "KYAE Grant Renewal Submission."

All required sections must be submitted together in the same e-mail message in their original file format (Word or Excel).   DO NOT SEND FORMS IN PDF FORMAT.

The original grant signature page and campaign finance law compliance form, both signed by the authorizing agent (agency head), must be mailed hard copy (in addition to submitting them electronically) to:

Deadline

All application materials shall be submitted electronically as described above no later than 4:30pm on Tuesday, May 23, 2006.

The original grant signature page and campaign finance law compliance form with the required signatures must be mailed hard copy as described above, postmarked on or before Tuesday, May 23, 2006.

PLEASE NOTE: Your electronic submission is not complete unless all application materials are submitted electronically in one e-mail message in their original file format (Word or Excel).  If you do not receive e-mail notification of receipt of your materials within 3 business days of submission, please call Leah at (502) 573-1555, extension 116.
